Tauroursodeoxycholic acid (TUDCA) is a combination of ursodeoxycholic acid and a taurine molecule.
Tauroursodeoxycholic acid is more effective than ursodeoxycholic acid alone in increasing bile concentrations of ursodeoxycholic acid and in reducing liver enzyme levels. This enhanced effectiveness is thought to be due to the increased bioavailability of the taurine group. The process of conjugating ursodeoxycholic acid with taurine is the rate-limiting step, and taking tauroursodeoxycholic acid bypasses this limitation.
Gallstones (composed of cholesterol) can be treated with bile acids in a litholytic manner, dissolving them to a size small enough to be passed through the body.
Cholesterol gallstones make up approximately 75% of all hepatic gallstones (in Western countries) and are the only type that can be dissolved by bile acids.
Gallstones may become opaque, indicating their resistance to dissolution. This opacity is caused by calcium deposition within the cholesterol gallstone, making it resistant to the cholelitholytic effects of bile acids. Opaque gallstones may be black (composed of calcium phosphate and/or carbonate, insoluble bilirubin pigment, and cholesterol) or brown (composed of calcium bilirubinate, calcium palmitate, stearate, and cholesterol).
It is important to note that only about 10% of people with gallstones will respond to tauroursodeoxycholic acid. This treatment is suitable for individuals who cannot undergo surgery and have small (5mm or smaller in diameter) non-calcium gallstones, which are high in cholesterol.
The dosage and course of administration are individualized: for postoperative prevention with gallbladder preservation based on ultrasound results, or for treatment without surgery based on computed tomography results.
